We are often invited to round-table discussions at various levels of government to help share and influence changes in healthcare, participating in research done by the Toronto Rehab Institute to examine best practices in elderly care. In 2013, we were part of the Ministry of Health’s multi-partner initiative to develop a video on mobility for the elderly population, now available publicly.
CTGHG’s technology to support medication management has measurably reduced hospital re-admissions. We are also the first homecare organization to put iPads in our nurses’ hands. Yes. Closing the Gap Healthcare is accredited by Accreditation Canada, which means that CTGHG has met or exceeded minimum standards of excellence as defined by healthcare providers. The standards and quality of care offered by CTGHG offices are the same, regardless of location across Ontario. CTGHG has achieved a high standard of service quality and is constantly raising the bar in pursuit of excellence.

The best place to start is to determine whether you qualify for funding from your local Community Care Access Centre (CCAC). If you do qualify, your local CCAC will introduce you to a care coordinator and this individual will work on your behalf to see what needs must be met. Although your local CCAC may not assign Closing the Gap Healthcare to your case, you may request our services specifically.
