Closing the Gap Healthcare is a healthcare service provider, offering home care and clinic based healthcare services throughout Ontario and Nova Scotia. We operate 11 physiotherapy clinics and 4 nursing clinics throughout Ontario. We also work in schools, retirement homes, long-term care facilities, hospitals, and throughout the community.

Yes, but you must qualify for publicly funded care. Publicly funded home care is funded by the Local Health Integration Networks (LHIN) in Ontario, and Nova Scotia Health Authority (NSHA) in Halifax. We currently have contracts with these government bodies to provide home care services. First, you would need to speak with the LHIN or NSHA to determine if you qualify for publicly funded care. If you do qualify, you may specifically request to receive service from Closing the Gap Healthcare. If you do not qualify, you can pay privately for our services or through other funding options.

Yes. Our physiotherapy clinics are OHIP funded except for the Toronto clinic. This means that if you qualify for OHIP, your physiotherapy sessions are free. If you do not qualify for OHIP, you also have the option of paying privately or through other health insurance plans.

In order to qualify for OHIP, you must meet one of the following:

  • Age 19 or younger, or age 65 or older, with a referral from a Physician or Nurse Practitioner
  • Recently discharged from the hospital, with a referral from a hospital Physician requiring physiotherapy services
  • Eligible for funding under ODSP or OW, with a referral from a Physician or Nurse Practitioner
  • Publicly funded home care through the LHIN or NSHA
  • OHIP funded physiotherapy services
  • Veterans Affairs Canada
  • Extended Healthcare Plans from your employer
  • Insurance benefits
  • Workplace Safety and Insurance Board (WSIB)

If you do not qualify for any funding options, there is always the option to pay privately for any service offered by Closing the Gap Healthcare.

Home healthcare is a cost effective way to allow individuals to remain in the comfort of their own homes while preserving their independence. Home healthcare is an alternative to staying in a hospital, long-term care facility, or retirement home.

Home care can be short-term or long-term, from simple needs such as meal preparation, to more complex needs such as wound dressing or palliative care. Whatever your needs, Closing the Gap Healthcare is here to support you every step of the way.

The best place to start is to find out if you qualify for publicly funded care from your Local Health Integration Network (LHIN) or the Nova Scotia Health Authority (NSHA).

If you do qualify, you’ll be introduced to a care coordinator and this individual will work on your behalf to see what needs must be met. Although the LHIN or NSHA may not assign Closing the Gap Healthcare to your case, you may request our services specifically.

If you do not qualify, you can pay privately for our home care services. To learn more, please call 1-877-560-0202 and one of our care representatives will be happy to help. We are available 24/7, 365 days a year.

If you’re receiving care from Closing the Gap Healthcare and it is publicly funded by the LHIN, you can speak to your care coordinator to see if the publicly funded system will authorize additional hours or services, or you can also supplement by paying privately for additional care

If you’re receiving private care, just give us a call and let us know that you need extra hours.

We offer services for special language requirements depending on the availability of our staff and your area of residence. Some of the languages we currently accommodate include English, French, Mandarin, Italian and Punjabi.

Yes, all of our caregivers go through an extensive hiring process that includes criminal and background checks. We want to make sure that you feel safe and comfortable while letting one of our providers come into your home. Closing the Gap Healthcare is fully accredited with Accreditation Canada, a leading health care accreditation body in Canada. This means that a third party authority has assessed, recognized, and validated that the care we provide follows excellent standards and uses best practices. In 2013, we were the first home care organization to score 100% on all accreditation standards, and in 2017 we earned our second consecutive Accreditation with Exemplary Standing, the highest level of accreditation an organization can receive.
