at your home, in your community, at your workplace.
Research and innovation are at the core of Closing the Gap’s Healthcare initiatives. We partner with various academic and non-academic institutions to share data, knowledge, and service provision, participating in discussion across private and governmental organizations. Both in product development and service delivery, we have repeatedly demonstrated our commitment to innovation.
We are often invited to round-table discussions at various levels of government to help share and influence changes in healthcare, participating in research done by the Toronto Rehab Institute to examine best practices in elderly care. In 2013, we were part of the Ministry of Health’s multi-partner initiative to develop a video on mobility for the elderly population, now available publicly.
CTG’s technology to support medication management has measurably reduced hospital re-admissions. We are also the first homecare organization to put iPads in our nurses’ hands. Further, we are the first homecare company in Canada to implement the Cleveland Clinic’s world-renowned Start with Heart program – an initiative to revolutionize customer service in healthcare.

The World Health Organization (WHO) developed the concept of “closing the gap” in a 2005 document that discusses the importance of closing the gap between knowledge translation strategies and policy transformations. This reflected our founder’s dream of a client-centred healthcare system supported by evidence-informed policies.
